Flexible in my approach in working in a person centred, solution focused way to enable and support adults to become self-reliant with short term/long term support therefore reducing the pressures on their families and adult social services when they are unable to manage their daily living requirements due to their mental health and/or cognitive disabilities.

Loneliness, mental health support, debt management, support with correspondence and benefits will be a few of the support that will be provided along with psychosocial assessments will be completed at the start of the support to determine what areas to focus on, with regular reviews. Support will be person centred and tailored to the needs of the individuals and their family.

Identify potential crisis areas to either work in a preventative way or to signpost to specialist services and/or report safeguarding concerns.
