Purpose of the Strategy Working Group: This working group will develop the strategy that will guide C4PC’s efforts in the coming months and beyond – and as importantly the plan that will ensure that we deliver this strategy together.

Meeting Schedule: The Strategy Working Group will hold monthly meetings via Teams from September to November. The first meeting is scheduled for September 11th at 10.00 -12.00. In this initial session, we will review evidence of barriers to personalised care for people living with long term health conditions, agree our strategic priorities for the next two years, begin to plan how we deliver those priorities and allocate responsibilities among members .
